Columbus Pain Center would like to welcome you to our practice. We understand what it's like to live in pain and realize that it can be a challenging and difficult road to recovery. Dr. Daniel H. Serrato and the entire staff are committed to patient care. We provide you with patient treatment plans to assist in reducing your chronic pain which can be associated with several conditions.
Patients are usually referred by their primary physicians. On our site, you will receive all of the necessary patient registration forms. Once you have completed these forms, please mail or fax them to us as soon as possible. We must have this information prior to your appointment so we can verify your insurance coverage and assist in the registration process on the day of your first appointment with us.

Along with the patient registration forms, you must complete a Release of Information form. Dr. Serrato must have a copy of your medical records from physicians you have seen or radiographic tests you have have taken in the past five years. These records must be received prior to your appointment. A separate form must be signed and completed for each physician or facility you have been treated or tests performed.
